Crit-Cases
Case 01
A 43 yr old farmer presents to your hospital with dyspnoea, and palpitations. On examination he is awake, respiratory rate 24 breaths per minute, heart rate 110 bpm, blood pressure 100/60 mmHg, with bibasal creps to midzone.
Case 02
You are asked to see a 74 year old man on the ward. He is drowsy with shortness of breath. His heart rate is 28 and his non-invasive blood pressure 63/35.
Case 03
You are called to see a 24 year old on labour ward. She is known to have hypertension. She is 35 weeks pregnant and is fitting.
Case 04
An 18 month old girl is brought to emergency in a peripheral hospital. She is limp, pale and unresponsive. Her mother states she has been feeling unwell for a few hours.
Her initial observations are : Heart rate 140, BP 70/46, RR 28, Sats 94% on room air. Her temperature is 35.9 degrees celcius. Capillary return 4-5 seconds. She flexes and groans to pain, but does not open her eyes.
Case 05
A 65 yr female is admitted to your ICU 10 days after elective cardiac surgery with sepsis secondary to sternal wound infection and a widespread rash.
Case 06
A 22 year old female is involved in a MVA. Her initial observations are BP 85/50, HR 135, Sats 94% (100% via NRBM).
Case 07
A 67 year old male is brought to ED via ambulance an hour after an overdose of metoprolol, quantity unknown. He is delirious and not following commands. His blood pressure is 85/45. He is known to have non-insulin requiring diabetes mellitus, hypertension and peripheral vascular disease.
Case 08
A 58 year old man presents to the emergency department of a peripheral hospital with severe central chest pain associated with a brief syncope, vomiting and excruciating pain radiating to his neck and throat. He is grey and sweaty. His initial observations demonstrate a blood pressure of 150/100, HR 110 Sats 97% and respiratory rate 28.
Case 09
A 57 year old male presents to the emergency department in a peripheral hospital with 2 hours of central crushing chest pain. He is sweaty and grey.
Station 11
A 47 year old male presents fitting. On arrival in emergency he has been fitting for the last 35 minutes despite 10mg midazolam.
Station 12
You are called to see a 34 year old on the ward with difficulty breathing. He had been admitted yesterday complaining of difficulty walking.
Station 13
A 63 yr old female has been transferred to your hospital following a motor-vehicle accident in which the car she was driving rolled off the highway at 90 km/hr. At the scene she had a GCS 3/15 and multiple facial injuries. She was intubated and on arrival is paralysed and sedated, with heart rate 110 bpm, blood pressure 110/60 mmHg, SpO2 99% on FiO2 60%.
